ANALISIS EFEKTIVITAS-BIAYA TERAPI PPI, H2RA, DAN KOMBINASI PADA PASIEN DISPEPSIA RAWAT INAP

Abstract


Dyspepsia is a common gastrointestinal disorder that contributes substantially to hospital healthcare expenditures, particularly within national health insurance systems. Rational pharmacotherapy selection should therefore consider both clinical effectiveness and economic efficiency. This study aimed to evaluate and compare the cost-effectiveness of proton pump inhibitors, histamine-2 receptor antagonists, and combination therapy in hospitalized dyspepsia patients. A retrospective cohort study was conducted using medical record data of dyspepsia inpatients treated at a secondary referral hospital from January 2024 to June 2025. Clinical effectiveness was assessed using length of hospital stay, while direct medical costs were calculated from the hospital provider perspective. Cost-effectiveness was evaluated using Average Cost-Effectiveness Ratio and Incremental Cost-Effectiveness Ratio analyses. A total of 197 patients were included in the analysis. Histamine-2 receptor antagonist therapy demonstrated the lowest Average Cost-Effectiveness Ratio, indicating the highest economic efficiency, whereas proton pump inhibitors provided greater clinical effectiveness with shorter hospitalization duration but higher costs. Combination therapy resulted in increased costs without proportional clinical benefit. Histamine-2 receptor antagonists represent the most cost-effective option for routine inpatient dyspepsia management, while proton pump inhibitors may be reserved for patients requiring stronger acid suppression
